Appointment Day
On the day of your appointment, please plan on arriving 10-15 minutes early to allow yourself enough time to complete any required paperwork and navigate the check-in process. At your first visit, please bring:
- Demographic Sheet
- Medical History
- Privacy Notice
- Records Release Form
- Insurance Card
- Photo Identification
- Copayment/Coinsurance
If you forget to bring these forms with you at the time of your visit, the receptionist will provide you with a copy.
At subsequent visits, please plan to bring your insurance card, photo identification and copayment/coinsurance.
Billing & Payments
As a courtesy, we will bill your insurance company for the services we provide to you. However, it is important to remember that you are ultimately responsible for your bill. You may find it valuable to contact your insurance company prior to your visit to verify that our provider is participating in your insurance's network. If your insurance advises us that you are responsible for all or a portion of your bill, we expect you to pay any remaining balance within 30 days of receiving your statement.
If you have difficulty paying for your bill at the time of service, we may be able to create a payment plan for you. Please ask to speak to a Financial Counselor for more information.
We accept cash, checks, and most major credit cards.
If you have questions about a bill for your office visits or ultrasounds, please call Judy at 263-2173 x 14. If you have questions about your bill for pregnancy or surgery, please call Helen at 263-2173 x 28. If you need to set up a payment plan, call Diana at 263-2173 x 17.
Collections
While we would prefer to never send a patient's account to a collection agency, situations do arise whereas patients do not fulfill their commitment to make satisfactory payments for the services rendered. In these instances, we will turn patients' accounts to collections so that we may be reimbursed for the services we provide in order to remain open for the community. All patients are encouraged to maintain frequent contact with our financial counselors to ensure that they are meeting their responsibilities.
Prescription Requests/Refills
We request that you contact your pharmacy directly to advise them that you are in need of a prescription refill. The pharmacy will then fax us a written request that includes your name, type of medication and dosage which enables us to authorize your request in a more timely manner. Please be advised that we typically require 24-48 hours to authorize your prescription.
Test Results
We communicate test results to patients in a variety of ways: by phone, by mail, or at an upcoming appointment.
Medical Record Requests
To request your medical records, please complete the Medical Records Request Form and mail or fax it to the clinic. Generally, we require at least five business days to fulfill your request. If your need is urgent, we will do our best to accommodate your situation.
